Heartland Events - JVA Compliance Requirement
(The forms below must be filled out by Wednesday , January 3, 2024 or your team/s will be removed from this event.)
Club Directors please fill out and sign: Club Director Verification Form 2024
NOTE: Only need to fill this out one cover the entire 2024 Heartland Event Season.
Coaches Sign in sheet please please fill out sign: Heartland Coaches Sign-in Sheet 2024
NOTE: Only need to fill this out one cover the entire 2024 Heartland Event Season.
Heartland Events will be verifying that USAV team’s coaches are compliant by the following Actions:
- Send a screenshot of their current USAV roster as the coach would be listed there and noted as passing their Background Screen and Abuse Prevention Training Course.
- Participating club directors fill out and sign the attached waiver verifying the coaches that will be on the bench are Background Screen and Abuse Prevention Training Course.
IF AN EVENT IS CANCELED DUE TO WEATHER- We send an email out and also post it on this Heartland Website notifiying Cancelation by 6:00 AM the day of the event.
